Terms and Conditions
These terms and conditions shall apply to each enrichment program and/or specialty camp offered by third party vendors (“Camps”) and/or Mackintosh Academy. Your acknowledgment and acceptance of these terms and conditions is a precondition of enrollment and participation of your child in any enrichment program and/or specialty camp.
A. Manner of Acceptance: In order to enroll, Parent/Guardian must agree to each section of this Agreement, electronically sign the Agreement, and submit the program or camp fee due at time of the registration. Either party may terminate child’s enrollment in the program or camp at any time, for any reason or no reason; provided, however, that such termination shall not relieve Parent/Guardian of the payment and other obligations set forth in this Agreement.
B. Cancellation or Withdrawal: Enrollment in a program or camp helps Camps and Mackintosh Academy budget accordingly. Parent/Guardian agrees to pay the tuition/fee amount set forth in this Agreement. Cancellations or withdrawals will NOT relieve Parent/Guardian of the obligation to pay the full amount for the program or camp in accordance with the amount as stated in this Agreement. The cancellation policy is included with the description for each program/camp on the registration site.
Please note that Camps and/or Mackintosh Academy reserves the right to cancel any program or camp that does not meet minimum enrollment. Full refunds will be issued and you will be notified at least one week prior to the start date of program or camp.
To cancel your registration: Email firstname.lastname@example.org your full name, participant’s full name, program or camp that you are registered for, and the camp dates.Please note: The cancellation policy for Renaissance Adventures is stated on their website and supersedes the Mackintosh cancellation policy.
C. Admission, retention, and dismissal — Mackintosh Academy and/or Camps, in their sole discretion, shall determine if the conduct of the child or parent warrants dismissal. Mackintosh Academy and Camps have the right to suspend or dismiss any child as a result of his/her behavior or if, in the opinion of Mackintosh Academy or Camps administration or leader(s), the interests of the child, program, or camp would be best served by such action. I/We agree that we and the child will respect and be bound by the program or camp’s practices, policies, rules and regulations, as may be amended from time to time. I/We and the child further agree not to engage in any conduct or activities that may be disruptive or detrimental to the administrative, educational or extracurricular environment of the program or camp. Mackintosh Academy and Camps set high standards for open, direct, and respectful, parent communication. Parent/Guardian acknowledges and agrees to comply with the communication policies as a condition of placement. Suspension or dismissal, or any termination of this Agreement by Mackintosh Academy, Camps, or Parent/Guardian, will NOT relieve Parent/Guardian of the obligation to pay the full program or camp fee. Admission of child for a program or camp does not guarantee admission for any subsequent program or camp.
D. Required Forms - I/We understand that ALL forms, medications and records required by Mackintosh Academy and Camps are to be turned in to the School in advance of the program in which my child is enrolled.
E. Permission for Transportation - I hereby grant permission for my child to participate in all scheduled off campus field trips during program or camp session. I understand that I will be notified in advance of the dates and locations of these off campus trips. Should I choose for my child not to participate in a trip, I will notify Mackintosh Academy or Camps in writing at least 24 hours prior to the scheduled trip. I acknowledge that transportation of my child involves certain risks to my child and those he or she may come into contact with, including, without limitation, risks of accidents, personal injury, and damage to property belonging to my child or others, and I knowingly agree to assume any and all such risks. I release and hold harmless Mackintosh Academy, its officers, directors, employees and agents from any liability resulting from the provision of these activities, unless such injury is the result of gross negligence. I understand that Mackintosh Academy maintains an insurance policy to cover the anticipated risks of field trip activities and that this release does not apply to claims that might be made under either the trip policy (within that policy’s limits) or the general liability policy maintained by Mackintosh Academy (within that policy’s limits).
F. Medical Acknowledgement and Consent Treatment - I/We understand that Mackintosh Academy and Camps recommend that my child consult a physician before engaging in physical activity, and, if my child’s physical health is questionable, that we obtain medical clearance from a licensed medical professional. As a parent or legal guardian, I hereby authorize faculty and staff of Mackintosh Academy and Camps to pursue medical care in the event of an emergency. The permission may include transportation to and from a medical facility by calling for an ambulance, or by a faculty, staff, or Camps member. In so agreeing, I acknowledge that a conscientious effort will be made to contact me and my emergency contacts before taking action if the nature of the situation allows. I agree to accept any expenses incurred.
G. Miscellaneous Provisions
The undersigned, being a parent or guardian of the registered child, states that he and/or she has read this document in full before signing it and enrolling the child in a program or camp, has had the opportunity to consult with legal counsel, and understands that it is a binding legal obligation.
